Position Description:
The UofL Trager Institute Optimal Aging Clinic is seeking a compassionate and experienced Care Navigatorto join our multidisciplinary Primary Care and Behavioral Health team as a Clinical Care Coordinator III.
This role's focus is multi-faceted:
- Provide support in coordination with the interdisciplinary team to help establish strong workflows that focus on increasing conversations with patients 50+ about their brain health and dementia prevention.
- Support the prioritization of Medicare Annual Wellness Visits at the Optimal Aging Clinic. Develop protocols with the team to implement brain health focused care plans that support patients in addressing their individual needs.
The ideal candidate will have a strong background in a healthcare, social work or community health work setting, or geriatric care, with a focus on patient advocacy. This position will serve as a key liaison between the patient, caregivers, healthcare providers, and community services.
Key Responsibilities:
- Coordinate the development of workflows with the interdisciplinary team to implement new wellness initiatives.
- Conduct assessments and help develop personalized care plans in collaboration other OAC programs.
- Obtain KPI reports from Practice Manager and provide updates for the social work staff and leadership for tracking purposes.
- Assist with facilitating access to supportive services, such as community programs, home care, and financial/legal resources.
- Support communication and care coordination between primary care providers, specialists, mental health professionals, and community partners.
- Maintain accurate, up-to-date documentation in the electronic medical record (EMR).
- Participate in multidisciplinary team meetings and contribute to quality improvement initiatives.
Preferred Qualifications:
- Professional degree or certification as a medical or social services professional. Ex: MMA, RN, CCHW, MSSW, CSW.
- Minimum 1-2 years of experience in healthcare navigation, medical social work, or behavioral health.
- Work in geriatric and dementia related care.
- Experience working with older adults with cognitive impairment and their caregivers.
Skills & Competencies:
- Understanding of cognitive impairment and previous training on risk factors impacting patients' likelihood of developing dementia.
- Understanding of social determinants of health and previous training on their impact on a person's overall health outcomes.
- Strong interpersonal and communication skills, with a patient- and family-centered approach to communication and care.
- Ability to work effectively in a team-based clinical environment.
- Knowledge of local, state, and federal resources for aging, dementia care and behavioral health.
- Cultural competency and sensitivity to diverse populations.

Target Compensation Maximum:
$28.55
Target Compensation Minimum:
$19.03
Compensation will be commensurate to candidate experience.
