JOB DESCRIPTIONThe Clinical Pharmacist delivers comprehensive, patient-centered pharmaceutical care across a variety of specialty areas, including Behavioral Health, Critical Care, Emergency Medicine, Infectious Disease, Pediatrics/Neonates, Oncology, Inpatient Transitions of Care, Investigational Drug Services, and Opioid Stewardship, and other. Responsibilities include reviewing and optimizing medication therapy, performing medication reconciliation, monitoring therapeutic outcomes, managing adverse drug events, and providing patient and caregiver education. The pharmacist collaborates with interdisciplinary teams to develop individualized care plans, participates in clinical rounds, and provides drug information and pharmacotherapy recommendations. Additionally, the role contributes to quality improvement initiatives, policy and protocol development, and clinical projects. The Clinical Pharmacist conducts medication use evaluations and research, and ensures compliance with institutional, state, and federal regulations to support safe, effective, and evidence-based medication use.
General Responsibilities
- Reviewing patient medication regimens and providing recommendations to optimize therapy based on clinical guidelines and best practices.
- Conducting medication reconciliation to ensure accuracy and safety during transitions of care.
- Educating patients and caregivers on proper medication use, side effects, and adherence strategies.
- Collaborating with healthcare providers to develop and implement patient-specific care plans.
- Monitoring patient outcomes and adjusting therapy as necessary to achieve therapeutic goals.
- Participating in interdisciplinary rounds and providing pharmacotherapy recommendations.
- Providing drug information to healthcare professionals and patients as needed.
- Contributing to quality improvement initiatives related to medication use and patient safety.
- Documenting interventions, clinical assessments, and recommendations in the electronic health record.
- Staying current with developments in pharmacotherapy and evidence-based medicine.
- Serve as a preceptor and mentor for pharmacists, pharmacy residents and students as needed
Inpatient Transitions of Care/Internal Medicine
- Proactively contributes to the decision-making process and the development of patient-centered care plans
- Advise clinicians on drug selection based on current clinical guidelines to optimize drug therapy and provide responses to drug information questions
- Counsel patients on medications and provide discharge counseling
- Provide transitions of care services for patients throughout the hospital
- Contribute to system-wide TOC/IM-focused clinical initiatives including guidelines, policies, and formulary management
- Intervene on medication therapy including, but not limited to, renal dosing, anticoagulation choice, dosing, and monitoring, glycemic control, IV to PO interchange, and pre-admission medication reconciliation
- Participate in multi-disciplinary committees including Pharmacy & Therapeutics, Medication Management, Medication Safety, and specific committees to support the appropriate use of medication therapy within specific disease states
- Participate in quality, safety and other project work
- Conduct medication use evaluations and research projects in collaboration with other healthcare professionals to identify areas of medication use optimization with publication and/or presentation at local and/or national meetings
- Precept pharmacy residents and pharmacy students
QUALIFICATIONS
Education/Experience
- Graduated from accredited school of pharmacy required
- Current NJ Board of Pharmacy license required
- PGY1 or PGY2 pharmacy residency training OR 2 years of clinical pharmacy experience in the assigned area

ABOUT THE TEAM
Atlantic Health System Pharmacy Departments service multiple outpatient areas, including several infusion centers, retail pharmacy services, and health clinics. At Morristown Medical Center and Newton Medical Center we have full-service retail pharmacies. They provide prescription medication services to patients and the local community in a convenient location on the first floor of the hospital, inside the Corner Store Gift Shop at Morristown and behind the coffee shop at Newton adjacent to the main entrance. A convenient service provided in our retail pharmacies is having your prescriptions you receive at discharge filled and ready for pick-up before you leave to go home. That means the discharge process will be a little easier for you and your family or friends since there is no need to stop at your retail pharmacy on the way home. The Pharmacy Department also works with physicians and nurses to ensure that medications used at Atlantic Health System are safe and effective. Pharmacists are integrated into your care teams to provide real time medication information and help you get the most benefit from your medications.
