The Community Health Worker (Lung Health Navigator) combines community outreach & engagement, and patient navigation to improve access to lung cancer screening for individuals at high risk. While serving all individuals, the Lung Health Navigator places special emphasis on populations disproportionately affected by lung cancer, including individuals from American Indian/Alaska Native communities. As part of a collaborative team, the Lung Health Navigator identifies people at high risk for lung cancer, determines eligibility for screening, and provides navigation support to coordinate screening and follow-up across community clinics, Fred Hutch Cancer Center, and partner organizations. Key responsibilities include outreach and navigation support to help individual address barriers to screening and ensure timely follow-up to support optimal care.
Responsibilities
Navigation and Coordination
- Review dashboards and rosters daily and reach out to patients to schedule baseline, annual lung cancer screens and follow up imaging.
- Conduct eligibility screening for individuals identified as high risk for lung cancer and navigate them to lung cancer screening.
- Guide patients through the lung cancer screening process, educate them on screening, schedule appointments, assist with arrivals, and ensure timely follow-up for those due or overdue.
- Provide logistical support and address patient concerns, including parking, directions, transportation, co-pays, insurance coverage, and other navigation needs.
- Provide referrals to commercial tobacco cessation treatment resources and complete patient notes and medical record documentation.
- Interface with medical staff from the lung cancer screening program (APPs and MDs) as necessary to ensure appropriate follow-up care.
Community Outreach & Engagement
- Deliver lung cancer screening outreach using culturally and linguistically appropriate methods to meet the needs of high-risk populations, including:
- Community events
- Community, patient and provider education
- In-service presentations for health-care teams
- Track and report program metrics to support program planning and weekly/monthly reporting.
- Maintain an up-to-date inventory of community resources and educational materials.
- Performs other related duties as assigned.
Qualifications
Required:
- High school diploma or GED equivalent
- One year of work experience in a clinical setting.
- Basic proficiency using Microsoft Office Suite (Outlook, Word, Excel)
- Strong customer service skills
- Organizational and time management skills
- Experience in community outreach and engagement
- Ability to work independently as well as collaboratively within a multidisciplinary healthcare team
- Ability to communicate clearly and professionally verbally and in writing, including in-person, by phone, and in public speaking settings
Preferred:
- BA, BS or MPH
- Tobacco treatment specialist certification
- Basic skills with electronic health records (EHRs), patient registries, or data tracking systems
- Patient navigation, care coordination, case management, and community health outreach
- Ability to work in a fast-paced environment while prioritizing and managing concurrent responsibilities
- Familiarity with healthcare systems, referral pathways, and screening
- Experience with chronic disease management and/or tobacco cessation services
The hourly pay range for this position is from $29.77 to $42.39 and pay offered will be based on experience and qualifications.
This position is not eligible for H-1B sponsorship at this time.
