JOB TITLE: Clinical Team Manager

JOB SUMMARY:
Responsible for coordinating and directing the clinical activity of an Atrio Home Health clinical team. Provides guidance to, and direct supervision of nursing, therapist, home health aides, and social services, along with evaluation of clinical competency and performance. Ensures client directed care, quality of clinical services, compliance with corporate standards and federal and state regulations. Assures appropriate utilization of care providers and management of actual or potential risks to Atrio Home Health.
PRINCIPAL DUTIES & RESPONSIBILITIES:
1. Manages the day-to-day activities and personnel of an Atrio Home Health clinical team. Assigns new client admissions and oversees clinical self-scheduling processes, ensuring equal distribution of client assignments and weekend/holiday scheduling. Reviews clinician schedules daily for productivity and care plan appropriateness. Validates completion of client visits including documentation submission.
2. Reviews staff documentation, including OASIS, to ensure compliance with agency policy, regulatory guidelines, and performance improvement measures. Collaborates with the quality review clinician regarding potential OASIS errors to insure appropriate delivery (level and type) of care.
3. Serves as a clinical and administrative resource to members of the team. Assists clinicians with scheduling of “overflow” client visits as indicated. Performs one on one case conference reviews at least every 15 days and mentors staff appropriately. Provides assistance as necessary for field clinicians; serves as back up for return calls from physician offices, labs, or clients when clinicians are making field visits. Performs client admissions only when indicated by adverse staffing situations. Facilitates and participates in team conference with all disciplines as requested.
4. Monitors quality of care provided, accuracy and completion of charting assuring compliance with regulatory and reimbursement requirements, through evaluation of comprehensive assessments and observation of practice. Ensures clinicians complete OASIS corrections timely, monitors error report summaries by clinician, assists with education of clinicians regarding accurate completion of OASIS and care plan, identifies educational needs and necessary disciplinary action as indicated.
5. Supervises and trains, assures adequate staffing level, evaluates employee performance and handles employee relations issues. Ensures that all clinical staff members are appropriately oriented and educated on an ongoing basis to safely assume their job responsibilities.
6. Monitors outcomes, productivity, and caseloads; holds staff accountable to goals.
7. Does initial investigation of any clinical issues and concerns related to services or staff. In collaboration with Director of Clinical Services, addresses and handles resolution. Communicates with Director regarding concerns and/or problems warranting further attention.
8. Ensures appropriate administration and utilization of tele-monitors by team members directs clinicians in use of tele-monitoring, ensures placement and use of tele-monitors by clinicians at time of client admission, and requires clinicians to review biometric transmission data and schedule home visits accordingly
9. Performs or delegates field supervisory visits with clinicians at least annually to validate compliance with policies, clinical procedures, and infection control processes. (See also Rehab Program Supervisor job description.)
10. Functions as a key member of the Atrio Home Health’s Management Team and develops program goals with the management team.
11. Serves on various inter-agency and organizational committees; performs related tasks as required.
12. Keeps abreast of changes in home health regulatory and accreditation requirements. Communicates changes in requirements and quality issues to assure accuracy and integrity of clinical data.
13. Works with the Administrator and Director of Clinical Services in planning and implementing program goals.
14. Participates in policy and procedure development.
15. On-call availability required on rotating basis, to handle work problems, emergency situations, etc
16. Is compliant to Atrio Home Care and Atrio Home Health’s policy and procedure standards, including HIPAA privacy standards.
17. Participates on various committees; performs related follow-up tasks and duties. Facilitates or attends staff meetings and inservice programs as required.
18. Performs job duties in accordance with the mission and values of Atrio Home Care.
19. Understands client rights and incorporates them into daily practices.
20. Supports and models by conduct the Atrio Home Care Code of Ethics and Conduct to clients, employees and the public.
21. Meets Atrio Home Care and HIPPA standards for confidentiality.
OTHER DUTIES AND RESPONSIBILITIES:
1. On-call availability required on rotating basis, to handle work problems, emergency situations, etc.
2. Performs other duties as assigned.
JOB QUALIFICATIONS:
1. The job requires knowledge normally acquired through a Bachelor’s Degree in Nursing.
2. Two to four years of geriatric nursing experience is required. At least 2 years home health nursing experience is required.
3. Licensure as an RN in the State of Michigan is required.
4. COS certification is preferred.
5. Employee must have reliable means of transportation.
6. Proof of a valid Michigan driver’s license and vehicle liability insurance is required.
7. High-Speed internet connection is required.
SPECIAL SKILLS & ABILITIES REQUIRED:
1. Ability to read, speak, write and understand the English language.
2. Interpersonal skills necessary to effectively communicate with all levels of staff in the exchange of information.
3. Skills necessary to provide guidance and review the work of assigned staff.
4. Computer skills necessary to maintain required documentation.
5. Ability to meet Atrio Home Care’s standards for attendance.
6. Ability to meet the general health requirements as set forth by the policies of Atrio Home Care, which includes a physical examination, TB testing and seasonal flu shot.
MENTAL & PHYSICAL DEMANDS:
1. Pressures related to meeting deadlines and working on tasks requiring concentration and attention to detail.
2. Regular exposure to mental fatigue resulting from reading detailed documentation for extended periods of time.
WORKING CONDITIONS:
1. Work is split between a normal office environment and client environment with minimal exposure to infectious conditions and communicable diseases when using standard precaution methods.
