Description
BASIC FUNCTION:
The Community Health Worker (CHW) will be responsible specifically to mitigate SNAP and Medicaid benefit applications due to loss based on HR1 Statutory program changes in work requirements. Will be able to serve as on-site engagement with our Food Pantry clients, Mobile Farmer’s Market Truck, and Farmer’s Market and assisting with overall Case Management basic needs.
Requirements
Utilize Tribeware system to collect client information; document client’s current SNAP and/or Medicare benefit status.
Engage clients through an intake process, conduct a pre-assessment, develop a client service plan and document with case notes.
Assist clients who have been removed from SNAP and/or Medicaid benefits and identify barriers.
Participate in outreach activities to identify clients who are on the verge or have lost SNAP and/or Medicaid benefits.
Utilize community partnerships to assist clients in gaining services.
Assist applicants or reapplicants with the creation of a My Account through www.connect.ct.gov, reviewing necessary documentation required, and provide follow-up to ensure the applicant’s SNAP and/or Medicaid status.
