Minimum Qualifications
Bachelor's degree in a related field or equivalent
Preferred Qualifications
UHC Preferred:
• Master's degree in Counseling, Social Work, Marriage and Family Therapy, Psychology or a related field.
• License eligible in the state of GA. Associate licensed obtained within 6 months of start date.
• Skill and/or interest in preventive psychological education and outreach programs.
• AHA Heartsaver CPR / AED certification required. Can obtain after start date.
• One year or more of supervised experience in clinical case management with individuals whose age and psychological development are similar to those of our population.
• Full independent licensure within the State of Georgia by the date of first employment.
• Two years or more years in a setting that provides crisis services.
• Demonstrated history and experience working with individuals from traditionally underserved populations.
• Training and ability to provide telehealth services.
Position Summary
The Mental Health Case Manager is an essential member of the Integrated Care Team and will provide appropriate and individualized case management to students and eligible partners. This position manages the referral and tracking of clients sent to other clinics within the UHC , discharged to the community, and/or transferred to a higher level of care. The Mental Health Case Manager will help obtain past and ongoing treatment records and be an active member of the Case Disposition Team. The Mental Health Case Manager will also provide primary, secondary, and tertiary services to eligible members of the UGA community, within the UHC mental health care scope of service. These services may include group, individual, crisis interventions, outreach, and consultation. Licensed Clinicians provide mental health evaluations, formulation of conceptualization, diagnoses, treatment plans, and crisis intervention. Services are delivered in accordance with UHC / CAPS policies and procedures.
Knowledge, Skills, Abilities and/or Competencies
- Strong interpersonal skills and ability to work in a fast-paced work environment.
- Training and skills in providing telehealth services.
- Knowledge of disorders typically seen at a university counseling service.
- Knowledge of crisis intervention techniques.
- Knowledge of and skill using an EHR .
- Knowledge and experience using word processing and data management programs including Excel.
- Practice safe, effective and appropriate mental health care.
- Perform and document mental health diagnosis, treatment, education, and coordination of care.
- Manage own time and job responsibilities with sensitivity to the needs of patients and collaborating professionals.
- Demonstrate respect and understanding of differences in behavior and communication.
- Demonstrate consistent cooperativeness, courtesy, confidentiality, sensitivity, and tact to all student populations.

Case Management
- Coordinate mental health referrals
- Track clients referred to other UHC clinics and/or to the community using a registry
- Advocate on behalf of the client as needed to ensure timely access to care
- Act as liaison between UHC and community mental health providers to assist in obtaining past and ongoing treatment records as indicated.
- Assist UHC clinicians providing services to high risk clients and other critical liaison functions.
- Perform scheduled and walk-in screenings for students.
